Type 2 diabetes with neuropathy in both feet;
10 year history of depression with suicidal thoughts;
10 year history fibromyalgia;
10 year history high blood pressure
----------------------------

5. What limitations you have because of your problems.

Problems standing for any length of time;
problems walking for any distance;
difficulty climbing steps/stairs
problems concentrating and with memory
